Yes. Unlimited is $200 a year (save 12%) and Plus is $950 a year (save 20%), both billed through Shopify.
Through Shopify. Surveys installs from the Shopify App Store and the charge appears on your Shopify bill. Upgrade, downgrade, or cancel from inside the app at any time, and Shopify prorates the change.
